These forms are provided for your convenience. Please do not hesitate to call us if you have any questions about the registration process or our facility! For faster check-in, fax or email your completed registration forms to us at 936‑372‑3229 or prior to your pet's scheduled arrival. You may also provide the forms to us at check-in. Vaccination records must be received prior to check-in or we may not be able to provide services as scheduled. You may contact your veterinarian and request that they email or fax your pet’s records to us, or we will contact your veterinarian for you.

Pet Registration Form
To ensure we have current information about you and your pets, please complete the Pet Registration Form at your pet's first visit with us and update it as needed, or at least annually.

Medication Form
If your pet takes any medications, vitamins, or supplements that will need to be administered during his or her stay, please complete the Medication Form. Please bring all medications in the original prescription container, with the pet’s name, dosage and reason for the medication. Over-the-counter medications must be accompanied by a veterinarian’s dosage recommendation.

Authorization for Release of Records Form
If you would like us to contact your veterinarian directly to obtain your pet’s vaccination records, please complete the Authorization for Release of Records Form.

Lodging Agreement
Please complete the Lodging Agreement each time your pet is scheduled for an overnight visit with us.
